The Exercise Referral project is a scheme organised by Elmbridge Borough Council's Leisure and Cultural Services, which enables healthcare professionals to refer patients for supervised exercise:

In order to be accepted onto the exercise referral scheme, approval from your healthcare professional is needed, along with information regarding the reason for referral. All referring healthcare professionals need to be registered to the scheme. To check if a healthcare specialist is registered contact the |Health Development Officer
Discuss your suitability for referral onto the scheme with your healthcare professional. If your healthcare professional (GP, physiotherapist, hospital specialist) is registered to the scheme and happy to refer you they will give you a completed referral form explaining why you need to join this scheme. You need to then make an appointment for an assessment by calling the |Health Development Officer. When attending your assesment you need to wear loose and comfortable clothing with suitable gym shoes, and don't forget to bring your referral form!
