Vary or Transfer a Premises Licence or Specify a DPS

To Vary a Premises Licence:

Complete the |Vary Premises Licence application form and return with the |correct fee to the |Environmental Health & Licensing Division.

To Transfer a Premises Licence:

Complete both the following forms and return with the |correct fee to the |Environmental Health & Licensing Division.

To Specify a Designated Premises Supervisor (DPS):

To specify an individual as a DPS, complete both the following two forms and return with the |correct fee to the |Environmental Health & Licensing Division.
