Application Forms

Forms for making applications under the Licensing Act 2003 are available below or from the |DCMS website (Department for Culture, Media and Sport). Forms can be downloaded (PDF format), printed, completed and sent to the Environmental Health & Licensing Division together with the relevant attachments.

Note: If you visit the Civic Centre to submit a licence application, please do so between the hours of 10.00 am and 4.00 pm. We regret that we are unable to accept applications outside these hours.

Temporary Event Notices (TEN)

• |Temporary Event Notice information - used to authorise licensable activities lasting not more than 4 days (92 hours).
